Thank you for your interest in Florals Forever! Because I personally preserve your flowers myself, there’s a limit to how many pieces I can complete each week. For that reason, I ask that you complete this form as early as possible so I can confirm capacity. I am looking forward to working with you!

  1. Please reserve your spot by filling out the intake form below. 
  2. Once you have completed the intake form, please click here to fill out the Agreement Form. 
  3. I will confirm availability and give you an idea of how long it will take to complete your custom artwork.
  4. Send your flowers to me as described here so I will receive them no more than 3 days after your event.
  5. When your artwork is ready, we’ll arrange for pickup or shipping (there is a charge for shipping).

I understand that by purchasing my Florals Forever piece I will give the Artist creative liberty to design the bouquet preservation how the Artist believes will look best. “Customizations” will be considered, but not required to be followed unless talked through with Artist.

I understand that I am responsible for a $200 deposit in order to hold my reservation. I am responsible for paying in full for my order when the pieces are finished. I am responsible for an additional $50 for the last-minute no reservation fee that is due with the reservation deposit. 

I understand that I am responsible for the shipping or drop off of my bouquet to Pressed Floral’s shop.

I understand that the condition of the bouquet as it is received establishes the outcome of the final product, and if deemed unworkable in the condition in which it arrives, Florals Forever will cancel the order and I will receive a refund.

I understand that if my delivery service loses my package, Florals Forever is not responsible for the cost of delivery or the cost of the bouquet. I understand that once my block or frame pieces are finished there are no changes that can be made. 

I understand that during the preservation process, flowers do change color and can darken or fade.

I understand that not all flowers in my bouquet will be used due to shape, flower condition or lack of space in the frame. If replacement flowers are wanted I am responsible for the additional cost. 

I understand that any leftover flowers, pins, ribbons or accessories that do not make it into the preservation piece will not be returned.
